Liability Waiver and Parental Consent Form

Coach: Danielle Burnett

Activity: Youth Athletic Training (Flag Football/General Fitness)

Location: Gilbert, Arizona

1. Risk Acknowledgement

I, the undersigned, understand that participation in athletic training involves inherent risks of physical injury, including but not limited to strains, sprains, fractures, concussions, or heat-related illnesses. I acknowledge that these risks exist despite the safety measures taken by the Coach.

2. Waiver and Release of Liability

In consideration of being allowed to participate, I, on behalf of myself and my minor child, hereby release, waive, and discharge Danielle Burnett and her assistants, including any assistants or employees, from any and all liability, claims, or causes of action arising out of any loss, damage, or injury that may be sustained by my child while participating in training activities, whether caused by the negligence of the Coach or otherwise, to the fullest extent permitted by Arizona law.

3. Medical Authorization

In the event of an injury or medical emergency, I hereby authorize the Coach to seek necessary medical treatment for my child. I agree to be responsible for any costs associated with such medical care.

4. Physical Condition Representation

I certify that my child is in good physical health and has no medical conditions that would prevent them from participating in strenuous physical activity. I agree to inform the Coach of any allergies (e.g., bees, grass, medications) or chronic conditions (e.g., asthma) before training begins.

5. Arizona Climate Notice

I acknowledge that training in Arizona involves significant heat risks. I agree to provide my child with adequate water and sun protection. The Coach reserves the right to cancel or modify sessions due to Extreme Heat Advisories.
